Zusammenfassung
Für die Behandlung großer uvealer Melanome stehen neben Resektionsverfahren und Enukleation strahlentherapeutische (Brachytherapie) oder strahlenchirurgische Verfahren (Protonen, Helium-Ionen oder Gamma-Knife) zur Verfügung. Wir berichten über 3-Jahres-Ergebnisse an 100 Patienten mit uvealen Melanomen, die mit dem Gamma-Knife behandelt wurden.
Patienten und Methoden
Alle Patienten wurden in einem einmaligen ambulantem strahlenchirurgischen Eingriff mit dem Gamma-Knife in Retrobulbäranästhesie behandelt. Die Dosis betrug 50 Gy, alle Tumoren befanden sich innerhalb der 50% Isodosis Kurve. Alle Tumoren waren groß (maximale Prominenz > 6 mm; Basis > 19 mm) oder lagen ungünstig (mehr als 90° Kontakt zur Papille oder Beteiligung des Ziliarkörpers).
Ergebnisse
73 Patienten wurden 1 Jahr, 33 zwei Jahre und 17 Patienten 3 Jahre nachbeobachtet. Die lokale Tumorkontrolle lag insgesamt bei 98%. Die Komplikationen waren eine Rubeosis iridis (12% der Patienten im ersten Jahr, 15% im zweiten Jahr und 6% im dritten Jahr) und die Entwicklung eines Sekundärglaukoms (14% der Patienten im ersten Jahr, 9% im zweiten Jahr und 6% im dritten Jahr). Die Gesamtenukleationsrate lag bei 6,3% pro Jahr. 61% der Tumoren lagen am hinteren Pol, 21% in der mittleren Peripherie und 18 % im Ziliarkörper. Der Visus der behandelten Augen war von der Lokalisation des Tumors abhängig. Nach durchschnittlich 1,9 Jahren Nachbeobachtung blieb der Visus bei 71% der Tumoren in der mittleren Peripherie stabil, in 19% der Tumoren im Ziliarkörper und bei 0% der Tumoren am hinteren Pol.
Diskussion
Für große Tumoren der mittleren Peripherie ergibt sich mit der Gamma-Knife-Behandlung ein nebenwirkungsarmes, bulbuserhaltendes Verfahren mit einer guten lokalen Tumorkontrolle. Gegenüber anderen radiochirurgischen Therapien ist die hier beschriebene Technik mit wenig Aufwand und ohne große Belastung des Patienten ambulant durchführbar.
Summary
Eye preserving surgery in patients with large choroidal melanomas includes local resection or radiosurgery. Brachytherapy can only be applied to small tumors in adequate localisation while radiosurgery (charged particles or gamma-knife) can be applied to all other tumors. We report 3-year results of 100 patients treated with gamma-knife radiosurgery.
Patients and methods
All patients received a single fraction gamma-knife treatment with a retrobulbar block. The radiation dose was 50 Gy and all tumors were within the 50% isodosis range. Tumors were inaccessible for Ru106 Brachy-therapy as they were either too large (max. height > 6.0 mm, basis diameter > 19 mm) or contact to optic disc exceeded 90° or infiltration into the ciliary body was present.
Results
Of the 100 patients, 73 were followed for 1 year, 33 for 2 years and 17 for 3 years. Local tumor control was 98% (2 recurrences). Complications following radiosurgery were rubeosis iridis (12% of patients after 1 year, 15%) after 2 years and 6% after 3 years) and secondary glaucoma (14% of patients after 1 year, 9% after 2 years, 6% after 3 years). Total enucleation rate was 6.3% per year. The tumors were found at the posterior pole (61%), middle periphery (21%) or ciliary body (18%). Functional results depended on tumor localisation in the eye: after a mean of 1.9 years follow-up visual acuity was stable (± 3 lines) in 71% of patients with a tumor in the middle periphery while 19% of tumors with infiltration into the ciliary body and 0% of tumors at the posterior pole showed stable visual acuity.
Discussion
Garnma-knife-radiosurgery offers a well tolerated, eye preserving treatment for patients with large choroidal melanoma in the middle periphery. Comparing our method to other radiosurgery therapies we can offer a fast technique which only requires a single visit.
