Summary
Processing and transfusion of collected autologous blood is routine in up-to-date surgery. So far, little attention has been paid to the fact, that certain substances appearing in revision arthroplasty may escape the filter system of a cell-saver. The authors report a case of surgery for a loose hip prosthesis with severe metallosis, where the blood processing system did not eliminate the titanium particles collected with the blood. Since only limited data exist concerning the long term effects of systemically applied titanium, cell-savers should not be used in presence of titanium debris.
Résumé
Le traitement et la transfusion de sang autologue est de routine actuellement en chirurgie. Jusqu’à présent, peu d’attention a été portée au fait que certaines substances apparaissant au cours d’une arthroplastie de révision semblent échapper au système de filtrage d’un récupérateur de sang. Les auteurs rapportent un cas de chirurgie pour une prothèse de hanche descellée avec métallose sévère, où le récupérateur de sang n’a pas éliminé les particules de titane collectées avec le sang. Puisque peu de données existent concernant les effets à long terme du titane par voie systémique, les récupérateurs de sang ne devraient pas être utilisés en présence de débris de titane.
