Fortschritte in der chirurgischen Behandlung des offenen Bisses

Zusammenfassung

Die bekannte Rezidivneigung nach der operativen Korrektur des skelettal offenen Bisses wird auf Grund von Literaturangaben sowie einer eigenen Nachuntersuchung dargestellt. Dabei zeigt sich, daß die Häufigkeit der Rezidive vom Ort und von der Art der Operation abhängig ist. Weiterhin besteht eine direkte Beziehung zur präoperativen Molarenrelation, wobei die Korrektur des Klasse-III-offenen Bisses die ungünstigsten Resultate hat. Bei zu erwartender stärkerer Rotation des Unterkiefers sollte deshalb der operative Eingriff in den Oberkiefer verlegt werden. Hierzu wird auf entsprechende neuartige Operationstechniken und Osteosyntheseverfahren hingewiesen.

Summary

The well-known tendency to relapse after surgical correction of skeletal open bite is presented, after a review of the literature and after a follow-up study of our own treated cases. It is shown that the frequency of relapse depends on the site and method of operation. Furthermore, the pre-operativeAngle classification must be considered; correction of Class III open bite is most unfavourable. Therefore, the operation should be carried out on the maxilla in those cases where a significant anticlockwise rotation of the mandible is expected. In addition, a new operative technique with bone plating fixation, is described.

Résumé

Les auteurs présentent, d'après des recherches personnelles et des données de la littérature, une étude sur la tendance connue aux récidives des corrections chirurgicales des open bite squeléttiques. Il ressort que la fréquence des récidives dépend de la localisation et de la nature de l'intervention chirurgicale. Il existe un rapport direct avec la relation molaire préopératoire; c'est la correction de la classe III des open bite qui donne le plus de résultats défavorables. A cause de la plus forte rotation mandibulaire prévue il faudrait transférer l'intervention au maxillaire supérieur. En outre, dans cet article, les auteurs présentent des nouvelles techniques opératoires et d'ostéosynthèse.
