Die Schwangerschaftsunterbrechung und die psychiatrische Indikation in Finnland

Zusammenfassung

In Finnland genügt die soziale Indikation für eine legale Schwangerschaftsunterbrechung nicht. Trotzdem benützen viele Psychiater die soziale Indikation als Grund für eine Schwangerschaftsunterbrechung. In Finnland haben die psychiatrischen Indikationen besonders in Helsinki und in der Provinz Uusimaa stark zugenommen. In Uusimaa ist ebenfalls das Verhältnis der Zahl der Unterbrechungen zur Zahl der Sterilisationen, als beispielsweise in der Provinz Kuopio. In Schweden erreichte die Zahl der legalen Unterbrechungen mit 50 pro 1000 Geburten im Jahre 1952 einen Gipfelpunkt. In Finnland beträgt die selbe Zahl heute 70, und für Helsinki sogar 230. Somit endigt in Helsinki jede 4. bis 5. Schwangerschaft mit einem legalen Abort. Anhand eines Vergleichs mit den Verhältnissen im Ausland ergibt sich, daß das finnische Abortgesetz ungenügend ist. Da 60% der positiven Gutachten zur Schwangerschaftsunterbrechung heute von Psychiatern ausgestellt werden, wäre es notwendig, daß die Praxis der psychiatrischen Begutachtung revidiert und vereinheitlicht würde. Dafür käme ein zentrales Organ, das noch zu schaffen wäre, eventuell in Frage.

In Finland social indications are not enough for legal abortion. Yet many psychiatrists write medical certificates on social indications alone. In Finland and, notably, in the province of Uusimaa and the city of Helsinki, abortions on psychiatric grounds have increased. Also, the ratio of abortions to sterilizations in Uusimaa is entirely different from what it is, e.g., in the province of Kuopio. In Sweden the peak year was 1952, in which over 50 abortions per 1000 births were undertaken. The corresponding figure in Finland was 70 and, in Helsinki, as high as 230 per 1000 births. Thus, every fourth or fifth pregnancy ended in legal abortion in Helsinki. The authors criticize the Finnish abortion law and review the literature published in other countries. In Finland the certificate is given in 60% of cases by psychiatrists; hence, they should endeavour to make the indications uniform. A central organ of some kind might be set up for the purpose.
