Pulse contour cardiac output as a clinically valuable tool for intensive patient monitoring. A critique of a recent paper

Pulskontur-Herzminutenvolumen als eine klinisch wertvolle Angabe für intensive klinische Patientenüberwachung. Eine Kritik auf einen neulich erschienenen Artikel

Summary

Positive as well as negative reports on pulse contour stroke volume or cardiac output measuring have appeared in vast quantity in the literature. A recent paper byVerdouw et al. in this journal is of the negative variety, but on insufficient grounds, since:

  1. 1.

    young healthy swine no not constitute a proper model for old age human cardiac patients

  2. 2.

    statistics and regression analysis, on which much of the negative conclusions are based, are used improperly

  3. 3.

    the electromagnetic flow probe cannot be used uncritically as the reference method, and

  4. 4.

    over 2000 comparisons, in three studies in man, of a new pulse contour method intended for intensive patient monitoring, lead to the conclusion that at least that method is reliable and has a clinically useful accuracy.

Zusammenfassung

Positive wie auch negative Berichte über Pulskontur-Herzschlagvolumen oder Überwachung des Herzminutenvolumens sind häufig in der Literatur erschienen. Ein in dieser Zeitschrift vor kurzem erschiener Bericht vonVerdouw et al. gehört zu den negativen Variationen, jedoch ungenügend begründet, weil:

  1. 1.

    junge, gesunde Schweine kein geeignetes Modell für ältere menschliche Herzpatienten bilden,

  2. 2.

    Statistiken und Regressionsanalysen, auf denen viele der negativen Schlüsse beruhen, nicht richtig verwendet worden sind,

  3. 3.

    der elektromagnetische Strömungsaufnehmer nicht kritiklos als Referenz verwendet werden kann und

  4. 4.

    mehr als 2000 Vergleiche (in drei Studien an Menschen) einer neuen Pulskonturmethode für intensive Patientüberwachung zu der Schlußfolgerung führen, daß die Methode wenigstens zuverlässig ist und eine klinisch brauchbare Genauigkeit hat.
