Zusammenfassung
Wir berichten über einen Fall von unilateralem Hydrozephalus des rechten Seitenventrikels bei „membranösem Verschluß“ des Foramen Monroi: Bei einem 2 1/2jährigen Kind trat mit einer Latenzzeit von zwei Monaten nach einer bilateralen Bronchopneumonie mit Prätoxikose ein Hirndrucksyndrom mit rasch zunehmender Bewußtseinsstörung ohne andere neurologische Ausfälle auf. Angiographisch wurde die Diagnose eines unilateralen Hydrozephalus bei Foramen Monroi Verschluß gestellt. Bei der Operation fand sich ein “membranöser Verschluß” des Foramen Monroi, jedoch kein diffus wachsender Tumor, wie das zunächst vermutet worden war. Die Behandlung, die zu einer raschen Besserung führte, bestand in der Fenestratio septi pellucidi in Kombination mit einem ventriculo-atrialen Shunt, der nach einem Jahr entfernt wurde.
Aus der zugänglichen Literatur haben wir 28 Fälle von „nicht tumorösen“ Foramen Monroi Verschlüssen mit konsekutivem unilateralen Hydrozephalus zusammengestellt. Die mögliche Aetiologie kongenital, entzündlich, posttraumatisch sowie nach Shunt-Operation wird diskutiert. Ferner werden das klinische Bild und die radiologische Diagnostik mit Betonung der angiographischen Zeichen besprochen. Auch werden die Möglichkeiten der chirurgischen Behandlung, v. a. auch die Fenestratio septi pellucidi dargestellt.
Summary
A case of dilatation of the right lateral ventricle due to membranous occlusion of the foramen of Monro is reported. A child aged two and a half years developed raised intracranial pressure together with disturbed consciousness, but other neurological defect, two months after an attack of bilateral broncopneumonia. The preoperative diagnosis of occlusion of the right foramen of Monro by infiltrating tumour was made angiographically. At operation the obstruction was found to be due to a membrane. The septum lucidum was fenestrated and a ventriculoatrial shunt was inserted. After a year the shunt was removed. Twenty eight cases of unilateral hydrocephalus due to nontumorous occlusion of the foramen of Monro have been reviewed, and the aetiologies have been discussed. Clinical picture and diagnostic procedures are reviewed. The authors discuss surgical treatment, and lay stress on fenestration of the septum lucidum.
