Summary
The intertrochanteric valgus osteotomy was first described by Pauwels (1936) and is still largely unchanged today for operative treatment of dislocations and pseudarthroses of the femur neck. The disadvantage is lateral displacement of the weight-bearing axis, which leads to valgus of the knee, dependent on the amount of intertrochanteric valgus osteotomy necessary. This detrimental effect can only be avoided by shifting the femur shaft laterally, corresponding to the degree of the intertrochanteric valgus osteotomy. In this way the leg can be lengthened more than by a simple valgus osteotomy and the shortening of the leg that usually occurs can be better compensated for. Consequently, we have operated on 29 patients in the last 3 years and used this method with good results.
Zusammenfassung
Die von Pauwels zur Behandlung der Schenkelhalspseudarthrose angegebene intertrochantere valgisierende Osteotomie, die im Prinzip auch heute noch unverändert durchgeführt wind, hat den Nachteil, daß in Abhängigkeit vom Ausmaß der erforderlichen Valgisierung eine u..U. erhebliche Lateralverschiebung der Tragachse and damit eine Fehlbelastung des Kniegelenkes im Valgussinne resultiert. Dieser negative Effekt läßt sich durch eine entsprechende Lateralisierung des Oberschenkelschaftes vermeiden, die gleichzeitig eine stärkere Verlangerung bewirkt, wodurch sich die praktisch immer vorhandene Beinverkürzung besser ausgleichen läsB. Wir haben unter Berücksichtigung dieser Faktoren in den letzten drei Jahren 25 Schenkelhalspseudarthrosen and 4 posttraumatische Varusfehlstellungen des Hüftgelenkes mit sehr gutem Erfolg operativ behandelt.
