101. Ist die Whipplesche Operation noch berechtigt?

Summary

Our experience covers 196 Whipple's operations. The indications were as follows: 60 times for papillary carcinoma, 45 times for carcinoma of the head of the pancreas, 87 times for severe chronic pancreatitis, twice for adenocystoma of the head of the pancreas, and once each for insuloma and Zollinger-Ellison adenoma. Mortality: papillary carcinoma, 6.6%; carcinoma of the head of the pancreas, 17.7%; pancreatitis 4.5 %; total mortality, 8.1%. The follow-up examinations show Whipple's operation to be absolutely justified, particularly in papillary carcinoma and in severe chronic pancreatitis. This is even more true as significantly increasing survival times are observed.

Zusammenfassung

Wir überblicken 196 Whipplesche Operationen. Indikationen: 60mal ein Papillencarcinom, 45mal ein Pankreaskopfcarcinom, 87mal eine schwere chronische Pankreatitis, 2mal ein Cystadenom des Pankreaskopfes und je einmal ein Inselzell-Adenom und ein Zollinger-Ellison-Adenom. Letalität: Papillencarcinom 6,6%, Pankreaskopfcarcinom 17,7%, und Pankreatitis 4,5%, Gesamtletalität 8,1%. Die Nachuntersuchungen zeigen, daß die Whipplesche Operation besonders beim Papillencarcinom sowie bei der schweren chronischen Pankreatitis absolut ihre Berechtigung hat. Dies um so mehr, als signifikant zunehmende Überlebenszeiten beobachtet werden.
