Summary

The caloric requirements of patients with serious craniocerebral injuries are ca. 4000–5000 cal per day. To cover these requirements by parenteral feeding large quantities of fluid would have to be infused, with the danger of cerebral oedema and overloading of the circulation. Moreover, certain substances (e. g. fats), if infused relatively too rapidly, would lead to a high serum concentration, exceed the renal threshold and no longer act on the metabolism. In contrast to parenteral feeding, tube feeding requires only 2500 ml fluid. A special composition of this tube feed, with a proportion of fat slightly below the ideal value, is discussed. For clinical practice it is suggested to start with purely parenteral feeding because of the usually present atony of the gastrointestinal tract and the associated enzyme disturbance, and to change gradually to tube feeding from the 3rd day onwards.

Zusammenfassung

Der Calorienbedarf bei schwerkranken Schädel-Hirnverletzten beträgt pro Tag etwa 4000–5000 Calorien. Zur Deckung dieses Bedarfs müßten bei parenteraler Ernährung große Flüssigkeitsmengen infundiert und damit auch die Gefahren eines Hirnödems and einer Kreislaufüberlastung in Kauf genommen warden. AuBerdem warden manche Substanzen (z. B. Fette) relativ zu schnell infundiert, zu einer hohen Serumkonzentration fiihren, die Nierenschwelle überschreiten and nicht mehr stoffwechselwirksam sein. Im Gegensatz zur parenteralen Nahrungszufuhr ist bei Sondenernährung lediglich eine Flüssig-keitsmenge von 2500 ml erforderlich. Eine spezielle Zusammensetzung dieser Sondennahrung — mit einem Fettanteil etwas unter dem idealen Wert — wird diskutiert. Für die klinische Praxis wird vorgeschlagen, wegen der meist vorhandenen Atonie des Magen-Darmtraktes and der damit vorhandenen Fermentstörung anfänglich rein parenteral zu ernähren and erst vom 3. Tag an allmählich auf Sondennahrung überzugehen.
