Zusammenfassung
Die Pneumonie zählt zu den häufigsten schwerenHaemophilus influenzae-Infektionen bei Erwachsenen. 60% der betroffenen Patienten mit durchH. influenzae Typ b hervorgerufenen Pneumonien sind über 50 Jahre alt, 30 bis 40% sind Alkoholiker, und 30 bis 40% haben eine chronische Lungenerkrankung oder eine andere gleichzeitig bestehende Krankheit. In der Mehrzahl der Fälle kommt es zu multilobulärer, fleckförmiger, diffuser und meist beidseitiger Ausbreitung in das Lungengewebe. Die Letalität derH. influenzae Typ b-Pneumonie schwankt zwischen 30 und 40%. Bei Patienten mit nicht-bakteriämischer Pneumonie, hervorgerufen durch kapsellose Stämme vonH. influenzae, ist die Beteiligung mehrerer Lappen und Ergüsse selten, die Mortalität ist niedrig.H. influenzae ist ein wichtiger Erreger der akuten Epiglottitis des Erwachsenen; ebenso scheintH. influenzae bei akuten Exazerbationen der chronisch-obstruktiven Lungenerkrankung (COLD) und der akuten Sinusitis eine wichtige Rolle zu spielen. Für die Resistenz gegenüber Ampicillin ist β-Laktamase-Produktion verantwortlich, die von R-Faktoren oder Plasmiden gramnegativer Bakterien kodiert wird. 1978 betrug die Gesamtresistenzrate für Ampicillin beiH. influenzae in amerikanischen Krankenhäusern 18%. Nach Exposition gegenüber mitH. influenzae infizierten Personen findet man im Nasen-Rachenraum verbreitetH. influenzae. Bei Kindern, die mit durchH. influenzae Typ b infizierten Patienten in Berührung kommen, beträgt das Risiko der Sekundärinfektion ungefähr 2,1%. Erwachsene, die engen Kontakt mit Kindern mit einer schwerenH. influenzae-Erkrankung haben, müssen auf das mögliche Risiko der Sekundärübertragung aufmerksam gemacht werden.
Summary
Pneumonia counts as one of the most frequent severeHaemophilus influenzae infections to afflict adults. 60% of patients with pneumonia caused by type bH. influenzae are more than 50 years old, 30% to 40% are alcoholics, and 30% to 40% have chronic pulmonary disease or other concurrent illness. In the majority of cases there is multilobular, maculate, diffuse and usually bilateral involvement of the pulmonary tissue. The mortality rate due to type bH. influenzae pneumonia ranges between 30% and 40%. In patients with non-bacteriaemic pneumonia caused by non-encapsulated strains ofH. influenzae it is rare for several lobes to be involved, there is little exudation and the mortality rate is low.H. influenzae is a significant pathogen in acute epiglottitis in adults and it also appears to play an important role in acute exacerbations of chronic obstructive lung disease (COLD) and acute sinusitis. β-lactamase production mediated by R-factors or plasmids of gram-negative bacteria is responsible for ampicillin resistance. In 1978 the overall rate of resistance ofH. influenzae to ampicillin in American hospitals amounted to 18%.H. influenzae are found in the nasopharynx of people exposed to others infected withH. influenzae. The risk of secondary infection in children who come into contact with patients infected with type bH. influenzae amounts to approximately 2.1%. Adults in close contact with children suffering from severeH. influenzae infections must be warned of the possible risks of secondary infection.
