Summary
Cardiac output was measured in 55 patients by the direct Fick method and an automatized thermodilution method. The results showed a good correlation (correlation factorr=0.87, regression equationy=0.91x+0.59). For the thermodilution method ice-cold normal saline solution as indicator substance was injected into the right atrium. The temperature-time-curve was measured by a thermistor located in the pulmonary artery. The area of indicator dilution was determined by electronic integration, the calculated cardiac output appeared as l/min in digital display.
This method qualifies for longterm control of ICU patients as well as for cardiologic diagnostic use because it fits the following conditions: 1. no toxicity of the indicator substance, 2. immediate results, 3. the unlimited repeatability of measurements within short time intervals allows a semicontinuous control of the cardiac output, 4. the technical equipment requires less expenditure than the standard methods for determination of cardiac output, 5. allows for simultaneous determination of the pressure in the right atrium, pulmonary artery and pulmonary capillary.
Zusammenfassung
Vergleichende Messungen des Herzzeitvolumens bei 55 Patienten mit Hilfe eines automatisierten Thermodilutionsverfahrens und mit dem Fickschen Prinzip zeigten eine ausreichend gute übereinstimmung der Ergebnisse. Der Korrelationskoeffizient der 55 Meßwerte warr=0,87 und die Regressionsgleichungy=0,91x+0,59. Bei dem von uns angewandten System wurde der Indikator (eisgekühlte Kochsalzlösung) in den rechten Vorhof injiziert und die resultierende Temperatur — Zeitkurve von einem in der Arteria Pulmonalis gelegenen Thermistor registriert. Die Fläche unter der Indikatorverdünnungskurve wurde elektronisch integriert und das errechnete Herzzeitvolumen in l/min digital angezeigt.
Das Verfahren eignet sich gleichermaßen zur Langzeitüberwachung von Patienten auf Intensivstationen und zur kardiologischen Funktionsdiagnostik, da einige wesentliche Voraussetzungen für diese Anwendungsgebiete erfüllt werden: 1. Völlige Atoxizität des Indikators „Kälte“. 2. Sofortige Verfügbarkeit des Meßergebnisses. 3. Praktisch unbeschränkte Wiederholbarkeit der Messungen — von anurischen Patienten abgesehen — und somit eine „semikontinuierliche“ Überwachung des Herzzeitvolumens. 4. Die instrumentelle Ausrüstung ist gegenüber den Standardmethoden zur Bestimmung des Herzzeitvolumens sowohl personell wie vom Kostenaufwand her wenig aufwendig. 5. Es besteht die Möglichkeit der gleichzeitigen Bestimmung von intrakardialen Drücken im rechten Vorhof, in der Arteria Pulmonalis und des Pulmonalcapillardruckes.
