The supratentorial astrocytoma; an investigation into classification and prognosis

Zusammenfassung

Die statistische Analyse des umfangreichen Materials berechtigt zu folgenden für den Kliniker bedeutungsvollen Folgerungen.

  1. 1.

    Die Einteilung der astrozytären Gliome des Großhirns entsprechend dem histologischen Befund in solche vom „Malignitätsgrad“ I oder II oder III gibt die Möglichkeit zu einer prognostischen Beurteilung. Die Zuverlässigkeit dieser Prognose mag für den Einzelfall hin und wieder problematisch sein. Statistisch gesehen ist sie überzeugend.

  2. 2.

    Die Gruppe von Tumoren vom Grad III hebt sich im schicksalsmäßigen Krankheitsablauf nur wenig vom Glioblastom ab. Ein deutlicher Sprung in der Überlebenswahrscheinlichkeit besteht vor allem zwischen der Gruppe II und der Gruppe III.

  3. 3.

    Die Astrozytome des Großhirns sind weder semibenigne noch semimaligne. Sie sind eindeutig maligne Tumoren. Vom Gesamtkollektiv haben nur 12% eine Überlebenswahrscheinlichkeit von 5 Jahren. Ohne die Gruppe I sind es sogar nur 3%. Bei der Gruppe I ist ein gewisser Anteil einer Sondergruppe: Astrozytom vom Kleinhirntyp (gutartiger „Bergstrand-Tumor“) zuzuweisen. Ferner ist noch zu berücksichtigen, daß fast 1/3 des Gesamtkollektivs, nämlich die innerhalb von 2 1/2 Monaten postoperativ Verstorbenen, bei der Errechnung der Überlebenswahrscheinlichkeit nicht mit berücksichtigt worden sind.

Summary

This work presents the statistical analysis of a case material of 1,495 cases with supratentorial astrocytoma, which is based on statistics collected from the neurosurgical clinics in Bonn, Düsseldorf, Cologne, Giessen, Munich, and Vienna.

From the results of the statistical analysis the following points are of special importance for the clinician:

  1. 1.

    The classification of the supratentorial astrocytic gliomata on the basis of the histology into Grades I, II or III makes a prognostic assessment possible. Admittedly the reliability of the prognosis in individual cases may be problematical from time to time, yet statistically it looks convincing.

  2. 2.

    The Grade III tumours differ very little from the Glioblastomata in the fateful course of the illness. A definite jump in the likelihood of survival exists above all between Group II and Group III.

  3. 3.

    The supratentorial astrocytomata ought to be designated neither as semibenign nor else as semimalignant; they are undoubtedly malignant tumours. From the total of all cases there is only a 12% probability of surviving for 5 years. Without the Grade I tumours this drops to as little as 3%. Among the Grade I tumours, a certain proportion correspond to a separate group, namely the “Cerebellar astrocytoma” (benign “Bergstrand” tumour). Finally, it must be noted that almost one third of the total case material, namely those who died within 2 1/2 months postoperatively, have been excluded from the calculation of the survival times.
