Summary
A total of 572 patients were treated 628 times for acute peripheral vascular occlusion. Arterial embolism was most frequent with 65% followed by arterial thrombosis and traumatic vascular occlusion. For differential diagnosis other causes had to be considered. The method of choice is extraction of the thrombus by Fogarty catheter or ring stripper. The results depend on the time between occurrence and operative treatment of the occlusion. Angiography is not necessary in a clear case of embolism, but essential when exact preoperative classification of the vascular occlusion is not possible.
Zusammenfassung
Bei 572 Patienten wurde insgesamt 628mal ein akuter peripherer Arterienverschluss behandelt. Der häufigste Verschlusstyp ist mit 65% die Embolie, gefolgt von der arteriellen Thrombose und den Gefässtraumen. Die Therapie der Wahl ist die chirurgische Entfernung des Gerinnsels mit dem Fogarty-Katheter bzw. dem Ringstripper. Die Prognose für den Behandlungserfolg ist wesentlich abhängig vom Zeitraum zwischen Eintritt des Verschlusses und der operativen Behandlung. In der Diagnostik ist bei gesicherter Embolie die Angiographie entbehrlich. Auf diese Untersuchung kann jedoch nicht verzichtet werden, falls präoperativ eine einwandfreie Artdiagnostik des Verschlusses nicht möglich ist.
