Summary
In 606 resections operative mortality fell from 10 to 1.2%. In distal resections in 66% good results were observed with a late mortality of 11%. A total mortality of 40% was found in total pancreatectomy, therefore this procedure was cancelled in 1980. In 289 partial duodeno-pancreatectomies combined with duct occlusion by Ethibloc our operative mortality was 1%. Late follow-up: Free of pain 88%, recurrence in only 2.1%. Preservation of endocrine function proven by a prospective study.
Zusammenfassung
Senkung der Operationsletalität von 10 auf 1,2% bei 606 Resektionen. Gute Spätergebnisse in 66%, Rezidive in 34% und Spätletalität von 11% bei distaler Linksresektion. Die totale Pankreatektomie führte zu einer Gesamtletalität von 40% und wurde deshalb ab 1980 nicht mehr eingesetzt. Bei 289 partiellen Duodeno-pankreatektomien mit Ethiblocgangocclusion betrug die postoperative Letalität 1%. Mediane Nachbeobachtung 4,8 Jahre: Rezidive in nur 2,1%, schmerzfrei sind 88%. Die endokrine Funktion fällt nach der Resektion um 50% ab, bleibt aber dann ohne weitere Reduktion konstant erhalten.
