Zur Chirurgie an Hufeisennieren

Summary

The authors discuss surgical problems as to the correction of urinary stasis in horseshoe -kidnies. The operative procedures should be individualized according to each situation. In early stages with only beginning hydronephrosis an isthmus-dissection, rotation of the kidney into a normal axis and nephropexy is sufficient. With progressive hydronephrosis an additional pyeloplasty will be necessary to completely drain the system. In special cases a plastic procedure alone at the ureteropelvic junction will do and the kidney-isthmus may be left untouched.

Zusammenfassung

Anhand von fünf Fällen besprechen die Verfasser die chirurgischen Probleme bei der Harnwegssanierung von Hufeisennieren. Eine typische Operation zur Korrektur der Urinabflußbedingungen gibt es nicht. In Frühfällen mit erst beginnendem Aufstau erscheint eine Isthmusdurchtrennung, Lagekorrektur der Niere und Nephropexie ausreichend. Bei ausgebildeter Hydronephrose wird eine zusätzliche Nierenbeckenplastik notwendig, um das Hohlraumsystem vollständig zu drainieren. In Sonderfällen kann eine ausschließliche Nierenbeckenplastik ausreichen und der Isthmus belassen werden.
