Behandlung schwerer Schienbeinkopfbrüche durch Patellaimplantation

Zusammenfassung

Schwere Impressionsfrakturen des Schienbeinkopfes lassen sich oft nicht reponieren, weil das Imprimat aus zahllosen kleinen Fragmenten besteht. Die Prognose dieser Frakturen ist daher schlecht. Wilson u. Jakobs haben 1952 einen neuen Weg gewiesen, indem sie vorschlugen, die gleichseitige Patella in den Defekt der Tibiagelenkfläche einzupflanzen. Über die Anwendung dieses Verfahrens wurde bislang im deutschen Schrifttum nur von Titze berichtet. Die Technik der Patellaimplantation wird geschildert. An Hand von vier eigenen Beobachtungen wird über Frühergebnisse berichtet.

Summary

Severely depressed comminuted fractures of the tibial condyle are very often hard to replace, since the imprimate consists of small fragments. The prognosis for such fractures is therefore unfavourable. Wilson and Jakobs have suggested in the year 1952 to implant the patella of the same side into the defect of the tibia joint. The German medical literature has only once reported about the application of this process (Titze). The technique of the patella-implantation is described. Personal observations in four cases are reported.

On the basis of satisfactory clinical results, the method is recommended as a supplement to the previously used therapy for severely depressed comminuted fractures of the tibial condyle.

De graves fractures par impression de la tête du tibia ne peuvent souvent pas être réduites, le déplacement se composant d'innombrables fragments. Il est donc difficile d'établir le pronostic dans le cas de ces fractures. En 1952, Wilson et Jakobs suggéraient d'implanter la rotule équilatérale dans la lésion du plan d'articulation du tibia. La littérature allemande n'a fait mention qu'une seule fois, jusqu'ici, de l'application de cette méthode (Titze). On explique la technologie de l'implantation de la rotule. Quatre observations de l'auteur sont communiquées. En raison des résultats cliniques satisfaisants, ce procédé est recommandé à titre de complément de la thérapie actuellement appliquée aux fractures graves de la tête du tibia.
