Unsere Erfahrungen bei der Hüftarthrodese mit Beckenosteotomie und Kreuzplatte

Summary

Indications, principles and surgical technique including postsurgical treatment are discussed for the arthrodesis of the hip by means of the technique of AO. Our own experiences are based upon 35 cases. The results are compared with similar procedures. The method requires a relatively large engagement on part of staff and costs and is by no means without danger. It represents, however, a progress because of the early mobilisation of patients without any cast immobilisation. Of prime importance is the exact comand of the technique and the knowledge of complications, which may associate here.

Résumé

Nous avons discuté les indications, les principes, la technique et le traitement post-opêratoire de l'arthrodèse de hanche par ostéotomie du bassin et ostéosynthèse sous compression par plaque en croix. Notre expérience clinique s'appuie sur 35 interventions de ce type. Les resultats aprés évolution ont porté sur des points de comparaison à peu prés identiques. A notre avis, malgré son prix élevé et les risques opêratoires, cette méthode est un progrès, surtout par la possibilité de mobilisation precoce, dans le traitement de la hanche par arthrodése qui reste malgré tout encore indiquée. Mais là aussi il faut bien connaître l'évolution et l'apparition possible de complications.

Zusammenfassung

Indikationen, Prinzipien und Operationstechnik einschließlich Nachbehandlung der Hüftarthrodese mit Kreuzplatte und Beckenosteotomie wurden besprochen. Die klinikeigenen Erfahrungen stützen sich auf 35 derartige Eingriffe. Die Ergebnisse der Nachuntersuchungen wurden mit ähnlichen Verfahren verglichen. Nach unserer Auffassung stellt diese relativ aufwendige und keinesfalls gefahrlose Methode besonders wegen der Möglichkeit der Frühmobilisation einen wesentlichen Fortschritt bei der Durchführung der noch immer berechtigten Hüftarthrodesen dar. Eine subtile Beherrschung des Verfahrens und die Kenntnisse der möglicherweise auftretenden Komplikationen sind jedoch besonders hier unerläßlich.
