Summary
8 patients with the Adie-syndrome and areflexia of the soleus muscle were investigated and compared with normal subjects: the motor function was normal, the electromyogram, the motor and sensory nerve conduction velocity were within the normal limit. The nerve action potential of the ischiadic nerve was normal. Recording the mechanically elicited afferents of mechanoreceptors of the soleus muscle, there was no difference between Adie-patients with areflexia and normal in fiber groups Ia, Ib and II. After novocaine infiltration of the soleus muscle and Achilles tendon the Ib-afferent volley decreases for both. In Adie-patients, there was a polyphasic H-wave with higher threshold and lower amplitude than in normal subjects. The normal recovery cycle of the H-reflex with paired stimuli demonstrated a balance of excitatory and inhibitory inputs at the spinal and supraspinal level; the posttetanic facilitation of H-amplitude shows a normal synaptic transmission of Ia-afferents. The electrically elicited silent periods in soleus muscle were normal and indicated normal functioning of the gamma-loop. The F-wave of plantar muscles was the same in Adie-patients as in normal subjects. The findings suggest that the absence of tendon reflexes in the Adie-syndrome may be due to delayed and desynchronized afferent volleys of the Ia-group in the proximal pathway near the spinal ganglion. This pathomechanism is probably caused by segmental and partial demyelination of muscles spindle fibres in this region, as described by Harriman and Garland [18] histologically in a patient with an Adie-syndrome. The mechanically stimulated afferents of soleus muscle spindles are not capable of producing a tendon reflex, but the more synchroneous response of fiber group Ia to electrical stimulation was observed as a polyphasic H-reflex-potential.
Zusammenfassung
Von 8 ausgesuchten Adie-Patienten mit teilweise oder vollständig ausgefallenen Beineigenreflexen wurden die jeweils am reflexlosen Bein erhobenen neurophysiologischen Untersuchungsergebnisse mit denen von Gesunden verglichen: die Motilität der Adie-Patienten war klinisch und elektromyographisch intakt. Die motorische und sensible Nervenleitungsgeschwindigkeit des N. tibialis sowie das proximal der Sitzfalte abgeleitete Summenpotential des N. ischiadicus waren in Latenz und Potentialform normal. Die durch mechanische Reflexauslösung erhaltenen Tibialisneurogramme von Mechanoreceptoren des M. soleus waren entsprechend den Fasertypen Ia, Ib und II differenziert und unterschieden sich nicht von denen bei Normalpersonen; Novocaininfiltration des M. soleus und der Achillessehne reduzierte die Ib-Afferenz. Die durch intranervale Reizung ausgelösten H-Reflexe wiesen an reflexlosen Beinen eine höhere Reizschwelle und eine niedrigere Amplitude als Gesunde auf und waren polyphasisch. Eine ungestörte Erholung des H-Reflexes bei Doppelreizen beweist ein stabiles Gleichgewicht bahnender und hemmender Einflüsse auf spinale wie supraspinale Neurone, eine posttetanische Bahnung des H-Potentials eine intakte synaptische Übertragung der Ia-Afferenzen auf Alphamotoneurone. Die normale F-Welle der Plantarmuskeln spricht für eine ungestörte Funktion polysynaptischer Verschaltungen im Rückenmark, die normale Innervationsstille tonisch innervierter Wadenmuskeln nach elektrischer Auslösung für ein ungestörtes System der Gammamotoneurone.
Während beim Adie-Syndrom mechanische Reizung der Spindelorgane und ihrer motosensiblen Afferenzen nicht zu einem Reflex ausreicht, löst elektrische Reizung der Ia-Fasern einen polyphasischen H-Reflex aus. Diese Desynchronisierung des H-Potentials entsteht wahrscheinlich durch Impulsdesynchronisation der Ia-Afferenzen infolge selektiver und partieller Demyelinisierung motosensibler Fasern in der Hinterwurzel oder rückenmarkswärts. Harriman u. Garland [18] beobachteten bei histologischen Untersuchungen im Rückenmark eines Adie-Patienten mit Areflexie eine rückenmarksnahe Faserdegeneration und vermuteten, daß es sich dabei um Fasern aus Muskelspindeln handele.
