Towards a European Classification of Procedures in Medicine (ECPM) — AIM-SESAME and the Dutch and German Approach —

Summary

The SESAME project, a part of the AIM programme, surveyed existing procedures and results of standardization on semantical aspects in the fields of primary health care, medical procedures and drugs. In Germany the ICD-9 is obligatory e. g. for the coding of causes of death, or in hospitals, for the coding of diagnoses. On the other hand in Germany and in the German speaking countries no single classification is used for the coding of medical procedures. On the basis of the preliminary work of SESAME, the Working Group on Surgery of the German Society of Medical Documentation, Informatics and Statistics (GMDS) proposed the translation and adaptation of the 1990 Dutch Extension (ICPM-DE) of the International Classification of Procedures in Medicine (ICPM), originally published by WHO in 1978, and using this for the German general classification of medical procedures. The Dutch authors consented to the German translation, the first version of which is now available. The Working Group expects this to be the first step in Germany towards a European Classification of Procedures in Medicine (ECPM). Its structure is one of the tasks in the draft workplan of the Technical Committee 251 on Medical Informatics (TC 251) of the European Committee for Standardization (CEN) and also a suggested proposal for an AIM main phase project (European Classification of Medical Procedures [Euclamep]). The paper discusses some of the international and German classifications of medical procedures and concentrates on the actual German development. In our opinion, the Dutch ICPM-DE is actually the best choice for Middle European purposes. To avoid further self-made developments in this field in Germany it would be useful, if the government or at least scientific bodies, would strongly recommend using in general for medical documentation a single classification of medical procedures.
